Root Causes with Fishbone Diagram Analysis

Still associated with the case presented in the previous article, Process Performance(for example the case of reducing the leakage of oil), Fish Bone diagram will facilitate the search for problems that cause oil leakage. This is an analysis process to determine in detail the roots of the problems occurred. The following image is a simple example to find the cause of the problem. Causes of the search there are two root root causes can be identified that Black light inspection procedures are not appropriate. And insufficient hydrostatic test pressure has been running during the process.

Improvement story should be filed

There are several documents that must be considered by the leaders and members to ensure continuity of process improvement to come. These documents should be stored in an orderly manner to ensure ease for other members to find and translate the improvements has been achieved. And further improvement will be easy to come by finding a gap that has not been touched before.

Visualization the Improvement Stories

Improvement is made at the expense of time and effort. In order to get the response and appreciation from others in the company, it must be documented as well visualized to the improvement storyboard. This is very important because: 1. To keep the team spirit in making improvement. Because by knowing the story improvement that has been on display so it looks clearly how achievement of which have been obtained and the possibility of further improvement can be made.

Control the Changes and Plan Next Improvement

Some of the questions above will prove that countermeasures could make the problem intractable execution and improvement achieved. Some methods used are 1. Develop a control method, making the procedure work, drawings, instruction, specification or other visual methods to control the process. 2.
